Applicants
are required to fill out an import application form or export application form from the Plant Quarantine Section, Crop
Protection Branch, Department of Agriculture, Jalan Gallagher, Kuala Lumpur.
Application forms are also available at all State Crop Protection Units within
each State's Department of Agriculture office.
The application forms duly filled, must be sent to the following address:
- Each application form must be accompanied by
a fee of five ringgit (RM 5.00) which is charged for each permit issued.
- A report of land or nursery inspection carried out
by officers of the State Crop Protection Units if these planting materials
are subject to post-entry quarantine inspection after importation.
